IT'S AS SIMPLE AS 1 2 3 and only takes about 15 minutes all together!!!
1.) GET YOUR CODE- This code is used to track you each time you take the survey and it never changes!!! Click here to get your code, then come back to this page to proceed with further directions.
2.) TAKE THE SURVEY- Just click on the link below to take the survey. Don't forget to insert your code for your name doesn't go anywhere on the survey!!! It's completely anynonmous! So be honest!
3.) SUBMIT- At the end of the survey there is a submit button which will automatically send the survey to our evaluators!
***NEVER TAKEN THE SURVEY BEFORE???--- unfortunately you won't be able to take it on-line for you need to have a parental permission waiver form signed, but you can show up to the next DICE meeting and take it then!!! The next time you can do it right on-line!!!
